I am not sure if this would help: I needed kind of health report to show to my school couple of months ago. Unfortunately, they did not accept. The school I am in is kind of not a fan of alternative medicine, so they did not confirm the papers Matia gave. It might be different for a work place, dont know. Hope you already found the answer.
