I've been on & off of it a few times - back on it.  I'm at work now but I have the packing slip at home (I think)  I will check and get back to you - I found it on line and ordered it a few months back.  I also recall, when Dr. put me back on it a couple of weeks ago that she said that I could get it through her.  I never checked as I already had plenty.  If you can't find it at Boma med - I'd call there front desk - they're always helpful
